Before you purchase, call the member services number on the back of your card. Ask specifically: "Does my plan cover a cranial prosthesis under Durable Medical Equipment?" Most plans that cover cranial prostheses fall under their DME benefit. If the first rep says no, ask to speak with a supervisor.

Get a prescription from your doctor

Ask your oncologist or specialist to write a prescription for a "Cranial Prosthesis" — that specific term is what reclassifies it from a cosmetic item to medical equipment in the eyes of your insurer. The prescription should include your diagnosis code (your doctor will know this).

Submit your claim

Download your insurance company's claim form from their website or app. Submit your superbill, your doctor's prescription, and the completed claim form together — by mail, fax, or their online portal. Keep copies of everything.

Follow up — and appeal if you need to

If you don't hear back in 2–3 weeks, call and ask for your claim status. If it's denied, don't accept it as final. Request the denial reason in writing and file an appeal. Ask your doctor to write a letter explaining how hair loss has affected your wellbeing and recovery. Appeals succeed far more often than people expect.
